Reduction in Amount of Insurance: The total amount of Life Insurance reduces by 50% at age 65, with an appropriate adjustment in premium.

Accelerated Benefit for Terminal Illness
This benefit allows you (and your spouse if covered) a one time option to receive up to 50% of the term life insurance proceeds, upon being diagnosed by a physician as having twelve months or less to live as long as you are under age 70.

OPTIONAL DOUBLE BENEFIT
Double Benefit for Accidental Death
If you die as a result of Injury covered under the Policy; and within 365 days after the date of the accident that caused the Injury; we will pay an additional life insurance amount after we receive proof of death. The Accidental Death Insurance Benefit will be paid in a lump sum in accordance with the Beneficiary Designation provision of the Policy.

Waiver of Premium: If you become Totally Disabled before age 60, and the disability continues for more than 9 consecutive months, you won’t have to pay your premiums for as long as the disability lasts and benefits are payable.

ELIGIBILITY & EFFECTIVE DATE
All association members up to age 65 who are US citizens and reside in the United States are eligible for this life insurance coverage

TERMINATION
Coverage terminates on the earliest to occur of: the date the policy is cancelled; the premium due date on or next following; 1) you are no longer an active member of the policyholder; 2) the date you reach age 70; or 3) you fail to pay any required premium; or the first day of the month on or next following the date you ; 1) request to cancel your coverage under the policy; 2) the date you enter the armed forces of any country (membership in the reserves or a call to active duty for 2 months or less is not deemed entry into the armed forces ); or the date the death benefit is paid.

DEFINITIONS
Actively-at-Work means a person is performing all the regular duties of his or her occupation on a full-time basis (at least 30 hours per week) at his or her customary place of employment. Injury means the Insured Person’s bodily injury which resulted directly and independently of all other causes from an accident which occurred while covered under the Policy. Total Disability means Your disability which: begins before your 60th birthday and while you are covered under the Policy; is caused by bodily injury or disease which prevents you: 1) from engaging in any occupation or profession for wage or profit; or 2) if not employed, from engaging in the normal and customary activities of a person of like age and sex in good health; and has existed continuously for a period of at least nine months.

ONLY ONE EXCLUSION FOR LIFE INSURANCE
This policy does not cover loss from suicide within the first two years of coverage.

Exclusions for Optional Accident al Death Benefit: This benefit does not cover death resulting from: intentionally self-inflicted injury, suicide or attempted suicide, whether s ane or insane (in Missouri while sane); war or act of war, whether declared or not; injury sustained while in the armed forces of any country or international authority; sickness or disease or medical or surgical treatment of sickness or disease, except pyogenic infection which occurs through an accidental cut or wound; injury sustained while committing or attempting to commit a felony; voluntary entry into the body of any intoxicant, excitant, hallucinogen or any narcotic or other drug or any gas, poison or poisonous substance or under the influence of alcohol; taking part in any race or speed contest. travel or flight (including getting in or out, on or off) in an aircraft or device which can fly above the earth’s surface: 1)if the aircraft or device is being used for test or experimental purposes, or by or for any military authority (aircraft flown by U.S. Military Airlift Command (MAC) or similar service of another country are not excluded), or for travel or is designed for travel beyond the earth’s atmos phere; 2) while serving as a pilot or crew member (or student taking flying lessons) and not riding as a fare paying passenger, or hang gliding or parachuting except where a parachute jump is necessary for self-preservation; or injury sustained while riding as a passenger on any aircraft owned, operated or leased by or on behalf of the policyholder.

SATISFACTION GUARANTEED
You have a 30 day right to examine the Certificate of Insurance. The certificate may be returned within 30 days of the effective date and all premiums, minus any claims paid, will be refunded.
